Patients Rights
- You have the right to considerate, respectful, and supported care.
- You have the right to personal and informational privacy. Discussion or consultation involving your care will be conducted discreetly and only with those involved in your care or monitoring its quality. All communications and records pertaining to your care will be treated confidentially. You can expect to be interviewed, examined, and treated in surroundings conducive to visual and auditory privacy. Those not directly involved in your care must have your permission to be present.
- You have the right to express spiritual beliefs and cultural practices that do not harm others or interfere with medical therapy.
- You have the right to approve or refuse the release of records except in cases required by law.
- You have the right to obtain from your doctor, complete and current information on your current diagnosis, treatment and prognosis in terms that you can reasonably be expected to understand. When it is not medically advisable to give such information to you, the information will be made available to an appropriate person on your behalf.
- You have the right to be involved in resolving conflicts about care decisions.
- You have the right to receive from your doctor, information necessary to give informed consent prior to any procedure or treatment. Except in emergencies, such information will include but not necessarily be limited to the specific procedure or treatment, the significant risks involved and probable outcome. Where medically significant alternatives exist or your request for medical alternatives, you have the right to know such information. You have the right to know who is performing the procedures or treatment.
- You have the right to change your doctor if other qualified doctors are available.
- You have the right to be informed of any investigational, research, or educational activities related to care, and can refuse to participate in any such activity without that refusal compromising usual care. You have the right to review that decision periodically.
- You have the right to refuse treatment to the extent permitted by law and to be informed of the medical consequences of your decision.
- You have the right to expect reasonable continuity of care. You will be informed by your doctor or his delegate of your continuing health care requirements following discharge.
- You have the right to complain about care, to have your complaints reviewed, and when possible, resolved without hampering care.
- You have the right to request and receive an itemized and detailed explanation of your total bill for services rendered.
Patients Responsibilities
- You have the responsibility to provide, to the best of your knowledge, accurate and complete information regarding your present symptoms, past illnesses, hospitalizations, immunizations, medications, Advance Directives, and other matters concerning your health.
- You are responsible for following the treatment(s) recommended by your doctor and as expressed by the doctor or other office personnel as they carry out the corresponding plan of care.
- You are responsible for promptly fulfilling the financial obligations of your health care. You are responsible for furnishing information and cooperating to expedite payment by a third party payer.
- You are responsible for following rules and regulations affecting your care and conduct.
- You are responsible for being considerate of the rights of other patients and office personnel.
- You are responsible for providing accurate and complete information about complaints.
- You are responsible for acknowledging when you do not understand a contemplated treatment course or care decision.
